I think fetishes are hardcoded (part of rework). And what they do is really down to the 2k events xD I only know futa, that some give items in the Init_ event, and that hairy means less shaving NPCs.
Get the patch from download link/forum. Make sure to apply it properly by reloading events (like updating mods).

Before starting a new game you have to enable mods in modules, or follow the refresh process for existing games (see forums). When I downloaded the game with the patch I just installed the mods and started a new game, perhaps the mods are not in my current save? Although when I press "Debug" and go to the "Modules" tab all mods are listed there...
Ugh! I see now the problem... its just the last thing you mention, they were double extracted... After fixing that the events seems to be reloaded correctly but I will start a new game just to avoid problems. Probably should repost this on the HHS forumbarteke22 Trying the preg mod and while it definitely works/worked with NPCs (currently 08/07/2010 and seeing newly pregnant characters out and about), I have not had any success with the PC impregnating or getting pregnant. I've taken people's condoms, set mine to off, cheated to give everyone a 100 fertility and set the multiplier to 1.25 (dropped down to .55 now), but even after weeks of trying with NPCs not a single one has gotten pregnant or gotten my character pregnant.
I will say that, possible UI bug, is that if you update the mod settings in the Map the NPC/PC multipliers are swapped (i.e. if you click on NPC, set multiplier to 1.25, then back out, NPC's value won't have changed but the PC value would become 1.25, and vice versa). I don't think that 'should' matter if both are at 1.25 hypothetically, but still.
I will also say that I've seen my character with the "Afterglow" status effect in debug, but it wears off and nothing happens. I can try to add save if that helps (erroring out when I try now), but it should just be the new game, the "service pack" with 4 updates, and 3 of your mods (the "everything requires this" mod, Andy+Annette, and Pregnancy). After 'doing the deed' with some NPCs, I'd rest for 12 hours intervals for multiple days, and yea nothing.

It doesn't seem like these overrides are properly applying to the "SpecialCharacters" like it did in previous versions.
For example, if I change the PopulationData\Parameters\StudentFemale.xml file to have a breast range of 1-4 it seems like all the generated people abide by this range, but none of the "SpecialCharacters" people do, such as Daisy Maddox who still shows a breast size of 6 or Ivana Yochild with 7.
That sounds like an intentional thing given these are special characters, wouldn't want to allow changes to them that have even the slightest possibility of breaking their chains.Afaik these are only meant for the generic population. SpecialCharacters are their own templates, though I suppose 'special' templates could be added at some point in the future to define min-max ranges.
I have 1.9.5 and don't remember seeing the special characters any different in any of my saves. I think you're onto something with that idea, but to add onto it, maybe set the flag to be tripped only when they have no more need to be a special character so as to avoid messing with anything that might bug them out.My last playthrough was a while ago but I'm pretty sure in 1.9.5 the population data ranges were still applied to even the special characters... I don't think I edited them all manually in-game. If I'm just remembering incorrectly then I think it would be a neat option if you could set a flag that would allow those parameters to apply even to special characters.

Sounds like you were using Lexville based on the 'change the way people look' part (Lexville allows you to change the way NPCs' bodies look, including what head they use). I mean, you CAN change clothes through corruption and school policy without Lexville, but I took it to mean what they look like aside from clothing, let me know if that is wrong.
