As far as I know it is completely random. What do you mean by 'triggering' it? Loading a save until you get it? What proof do you have that it really is as you say? You may just be having random bad luck.
Both scenes can trigger if the 2 girls are on the island, it's random.

If they haven't told you themselves, the only way to get the information is to use the Companion Mod. Right now, knowing their age, doesn't really have any impact on the game.
I thought Renee was Canadian (her skating history would point that way) and Heather Irish?
The info is from the Hotshots game, so I would say it's correct.
I don't think tiers is a concept Darkhound himself necessarily follows. It is more something of the followers. Search the thread as there has a lot of math been going on to determine tiers and to relate the content girls were receiving to their (patron) rank. Darkhound certainly is here and responding here as well. But the patrons provide him with the money for the hardware to do all this on. So he has to take patron preferences into account as well. But in the end it is all his decision. I think most people are fairly happy with them. You can't please all people all the time but he is making an effort.
