Does her sanity have to be a certain level? I've left her working him for two weeks at high sanity and it is still on 5. By the way thank you for the game. It's one of my favorites.

Personally, I feel you - replaying the game to see all the scenes is not my jam.
But it doesn't feel right for Maya to spend half of her day sucking dicks at the clinic then coming for a friendly threesome later the same day. The writing would be kinda schizophrenia-inducing at that point.
I don't get the part "orgasms are denied in any position the lock is in". She can orgasm when she's locked and at 1000 arousal only later in the game, to simulate the "pill" she's got, or the rules she's followed. In the beginning, if the lock is ON, she can only cum if a 1-time event is written that way - she's supposed to have orgasms sometimes during main-plot scenes.
I'm like a trash-panda, I have this massive dump of mods installed from a repack I found. I have no idea what's from where, and where to get more. I rarely add anything new to it.
A proper sex scene with Melissa was not something I wanted to do, to emphasize that Sam is kinda loyal to Esther. He only trains Gloria to achieve a goal. The reason I added the "Fuck Melissa" option was that I didn't want to be crucified by the mob for adding a slut-character and making zero scenes with her

Nope. Just make sure that she IS assigned. Check the evening camera. If she's being fucked the number should go up at the start of a new day until it's 13.

There are a lot of options to tweak your progress with different stuff.
Use the "older versions" menu.

So is there any way to, or a plan for the future to, tell the uncle to f off, or maybe put him in his place? Everything I've seen so far leads me to believe he's a complete moron that's barely running a half-dead gang, but every time the mc talks to him he acts like some big shot and mc is some bitch. Really makes me want to kill the uncle or take over his gang or something.
pretty sure his deals with adam walker are about the mc becoming the eventual head of the vargas family
I ended up really enjoying this game and the characters; Especially Esther. Part 1 being more of a visual novel with a lot of stale memes ended up somehow working into Esther's charm. Her and the MC's interactions, banter, etc. are all fun and enjoyable because of it. Also like the fact she's the only character you can't pimp. It's a shame she's somewhat sidelined for Gloria content, probably for the better as the trainer stuff is more interesting, and that Gloria's best scenes are with other men and not the MC.
