Masturbation is key if you want the inmates to fap themselves into submission. To get Karryn aroused enough to masturbate in combat, either sadism or some help from the inmates is usually needed. Aside from that, you'll want high mind and agility to stay in control. The main problem is that masturbation isn't all that reliable, you still need to be ready for regular combat whenever Karryn's not feeling like touching herself, and horny enemies will still be touchy, so you'll need to take care of them some other way.
Alternatively, if your slut level is still relatively low, you can Flaunt and keep beating on the inmates while they're jerking off. This isn't much of an option if your desires are rising too fast to be suppressed, though.

From starting titles, anything that gets you extra coin is good. Ambitious Experimenter is still king, I think.
Extra sensitivity is mostly up to your taste, with whole body and mouth being more of a pain than others. Vaginal sensitivity is pretty good if you want Karryn to stay a virgin - but only while you can keep that desire low.

That would not fix your problem. The only enemy type that has the paizuri preference, are Orcs and some Guard Types. No other enemy type will go for that by themselves. You would have to use the Sex Skill.

Agreed, though I still would not consider KP 'cerebral' in its approach.
