I'm not sure what was meant by "2x S", but there is no incest in this game. It's a story about MC's mother's friend letting MC move in her house. They end up doing mommy play, no actual incest tho.

S = sister and the MC has two of them; M = mom.

I always treated them as mom and sisters, ignoring the Patreon required no incest in their TOS.

There is no iPatch and RPGM AVNs are typically a bit clunky in if/how they allow you to change relationships 'in AVN' so play it as it is written or roleplay it - whatever works best for you.
