All I know is they said it won't be out as quick as their was, and they posted a preview screenshot on 4Chan of the main screen. The chat, prepare etc buttons were done not the time or dialogue window ones.
If you mean during sex you pick the action with the button, but actually do it by clicking on Sylvie. Otherwise not sure what it could be, unless it's one of the possible issues from extraction.

You can just start again for the beginning again. Shot guide on Sylvie not dying here, it is a bit strict on what you have to do. She will only get sick once, after that you don't have to worry and can do what you want.
as I prefer to do as much content in a game as possible Id prefer to kill her and get the bad ending before i like her then i can work to the other ends but if its like doki doki that's bad for me i hate when files break
She just dies and it kicks you back to the main menu. That is the only ending, and it's more of a punishment really.
I still plan on getting it I like to try and get the endings but if its fast like that I can speedrun it
