When the sister is sick you can't have sex with her.
That's why I think this scene is very rare to happen, I think it really only happens to people who create a new save and abuse the stats they give you by helping her bathe her when she's sick.
Another fact is that the first times you have sex with the sister and given her mana disease, she ends up sick the next day and requires care.

Was there a way to engage consensual sex without raping her? Its been a while since I last played this,but I remember there used to be an option to directly start sex without sleep molesting her....But will this option appear without rape?

So Inusuku does expect one to have woken the sister up at night at least five times minimum.
