This game seems to run the gambit of fetishes. I do have a question; are there any plans to allow the player to get the girls pregnant? I found emily, and also the one scene where serena does her fake pregnancy, but are there plans for more beyond that? Hell I'll settle for the MC getting gender swapped again and impregnated.

I think it gets the tag because of the two I mentioned. Emily joins you already pregnant, and serena can fake a pregnancy with magic. From what I've heard from other people, the dev doesn't intend to go beyond that. That is, save for one girl who wants to start a family but can't yet, but that might not happen.
