You can find it in bad end dungeon

No but there are some items that trigger an auto-defeat.

If you have the collar at the start of the twins battle its an auto-defeat. Same with the black charm if you go against Risty without burning them all in the fire. If you have just one in inventory it will be auto-equipped and straight to defeat. But nothing AFTER battles
