Why are the Mac downloads not being recognised as mac apps?

I'd offer to help, but I use PC. Have you reached out to the dev on Patreon? They haven't posted anything in forever, so it might be futile, but it could be worth a try.
is there a way to assign Space button to whatever Mouse1 does?
Space is "End Cutscene" by default (wtf???) but I don't see the way in controls to replace LMB with any keyboard key

The game's in Unity, so unless Milfarion happened to make such an assignment (I don't believe he did), the answer would unfortunately be no. Unlike Ren'Py, there are no out-of-the-box keyboard / mouse button shortcuts for Unity games. All of those have to be programmed in each individual game. From my knowledge of Milf's Resort, I don't believe there is any such shortcut at this point.
on the "I don't believe he did" part author certainly thought: "yep, people are here to skip scenes so let's assign this most frequently performed action to a convenient Space button":

Just because Ren'Py does this out of the box doesn't mean all games have to. I don't actually think it's nearly as bad as you say. LMB still advances just like in Ren'Py, for example. Would it be nice to use spacebar to advance a single piece of dialogue? Sure. Does it amount to gross incompetence? IMO no. But, ofc, that's just my own opinion.
man, it's not even customizable in controls. many things can be re-assigned but there's no slot to reassign LMB smh:

Possibly. Milfarion is another of those rare developers who never communicates anything and then suddenly up pops an update, even after a couple of years. I'm not holding out hope, but it is possible this is not actually abandoned. I'd recommend continuing to watch this thread and keeping a copy of your saves, just in case.

Someone really needs to take over this game and convert it to renpy game.

I doubt that will happen. Unless Milfarion has actually abandoned this, I assume they wouldn't want to let anyone else use their IP or convert it, since they've probably chosen Unity for a reason. (I've no idea what the reason would be, ofc., but maybe they feel the most comfortable with that engine.)
It's hard to tell whether this is still being developed when their pattern in the past has been release an update, then go radio silent for a year or more, then release another update. The fact that it will be 2 years in just a few months is definitely concerning, but that still doesn't mean they've ditched work on the game entirely. Personally, I would just like to see another update and have this continue.

The gaming engine is not Ren'Py (the orange label to the left of the game's name on this thread indicates it's Unity), so a rollback function is not native. Milfarion would have to program that in, which obviously he hasn't done. You can save in freeroam mode only; if you're in the middle of a cutscene, saving is disabled. If you're in freeroam, hit Esc on your keyboard and then pick from the menu. Note that you can load at any time, even during a cutscene.
