hH already has, an MC that is visually not 10 -14 years old, Ren'py rather than RPGM and non linear game play. ICSTOR knows his market and is pretty formulaic on his game design, i'd be surprised if he branched out too much further this game, though i think every one of those changes is an improvement i think ICSTOR is smart enough to take it slowly and bring his fans with him rather than make a break to something new and leave some behind.
As for the whole NTR thing i think it can break the story a little if the only sex going on is between the MC and the girls he meets, i'm not talking about every girl wantonly cucking the MC just that if for example you are having sex with a girl that was with multiple partners before the MC and there is no plot reason for her to stop i don't see why she should stop.
It really depends on the type of relationship or lack of one the MC has with each girl, Zuri and Suri for example, it's almost purely a transaction with the MC, no reason they shouldn't be screwing around with someone else for their own gain as far as i can see, however with Sara since she is madly in love with the MC there is no reason for her to be with anyone else, Caroline is somewhere between the two, she could find a boyfriend and the MC could win her away from him, it really should be character and story specific.

As for actually using NTR in a game, well using it as a "bad end", yep makes sense, using it as an incentive for the player to do certain things, like in BB where the whole point of the game was get rid of Eric to prevent NTR, then yes it's a design decision.
As for using NTR to give the player his comeuppance, it could be interesting, for example if you lied to Zuri and Suri they could seduce the father and brag to the son about it or perhaps even send the pictures to the mother to "homewreck" something like that.
Potentially the vengeful ex GF trope is something people haven't done much with yet to my knowledge but could be interesting, start in an NTR situation and bring her back in later, either winning, seducing or coercing her back depending on what the story demands.
As you say though people don't play these games for realistic repercussions and consequences, hell this is in no small part an incest game, you don't see the police breaking down doors and arresting the MC for molesting his family, or arresting the mother for molesting her son for that matter, i don't think that would fly as a game idea somehow.

in general the i would say that anything you can do against the dad, if it gives you sex scenes or not you should, in games like these the father is either absent, either being a sainted and dead dad or an absent asshole, if he's present he is either divorced or a problem to be overcome, screwing him over is what i expect will be considered the "better" option.
