You get it by using Triangular Chair and choosing Forced Organsm. Afterwards you will get a text from Chiho with the picture. But it is kinda buggy so you might not get the right picture.

anyone got a save that is after the onsen trip my game breaks whenever i try to run day 81
We can't save when we are in the hotel/onsen? What happens if I exit the game will I have to start from my latest save?

Well... that did not happen, in fact I am no longer able to trigger the last key event that leads to the onsen on my latest save. I will have to takes this L
Really weird desicion to not let players save in the onsen but it is what it is

It says there, you have to have sex for a whole week straight and every time you do that, you have to get on the phone and talk to your husband to fill the bar, but water with the exclamation marks(!) because if you accumulate 3 in a row you lose
