This confirms my suspicions that the throuple path is an exclusive one. I wonder what that entails for an Ian and Lena that agreed to experiment with other partners 
. Do they try to get Holly in on it? And would Holly agree to it? If not, would she still give her blessing for them to still experiment at the risk of feeling left out? My Ian and Lena are casual. Throuple happens with no choices and the discussion of MFM happens just fine.
I'm catching some of my other paths up before I continue my main save. I was wondering does Lena have to not be on the Ian path for this choice. As I did a speed run with cheat mode and still couldn't get it, so it wasn't due to a lack of attribute / skill points.

Yes. Either not hooking up with Ian in the first place, or broke up with him. Lena also has to show some interest in Holly earlier:
