Due to Patreon guidelines the creator needed to 'remove' all the incest and questionable content. The patch restores this, otherwise you're missing over 75% of the total content in this game. If you have the game downloaded and extracted you'll notice that there's a folder called "game," you'll notice that when you extract the patch you'll have a folder inside it also called "game." So just merge the patches "game" folder into the original download, and overwrite if it asks you to. There's also instructions how to do this on the lewdpatcher link and in the download for the patch.

To answer your other question, no, no BBC is in this game, the What If set in Africa very infamously uses an all white skinned lost tribe of men for its premise, and all canon NTRers are also white. The main character, who is also white, is also said to have a rare genetic condition that grants him a "sub-saharan" cock, but again he's white.

That the only reason why Sterling is the hero to begin with and Hunter the villain is because he's the POV character, and we justify it by his physical attributes vs Hunter's ugliness of aspect and character.
