Only at first. The game has hidden counters for each kind of sex (I think primarily to track which dialogue choices to spit out), so once you get the girl to do enough head, they start to vary their choices.

I've had them do the third level before. I don't think they've ever gone to deep throat, though, from that option. I think it may be possible, but each further option is just less likely.

There's a random element to it, but basically until they've given 5 blow jobs its mostly "lick it" and a small chance of "just the head". After 5 blow jobs its still mostly "lick it", a moderate amount of "just the head" and a slight chance of "suck on it". After 10 blow jobs its an even chance of all 4 options.

Regarding the question, if I was a small pp e-boy and I had a fanbase looking for that, I wouldn't get a dick implant to change from Lilliputian to Brobdingnagian.
