You buy it on the skill screen then achieve 200 on one of the skills. I think that for the cosplay is lewd... But I could be wrong.
It's just to let you know vigilance is too high. If it reaches 100 it's game over (unless you fulfill the conditions to unlock 3P and 4P).

You must downloaded the Zip archive, right? With right clic you can select to unzip it wherever you like. IT will take some time, don't worry. Then open the Game file.
