Imho, that "broccoli" thing look like it was inspired by an old movie, can't remember the title, where MC's mom said something like "Life is like a box of chocolates..." to him. Heck, even half of the first season of the "other game" looks like it was spied on from another old movie about pie. Can't say that it somehow makes the "other game" worse, same goes for the "roll" thing for SG.
Also, when you go with Bella in the gym, he seems to recognize her somehow

Maybe it's only me, but it looks like a vein cut on the flashback picture to me, for a while ago I thought that MC tried to kill himself, but it somehow doesn't fit up with the current plot.
The dialogue during the scene says something about his lung has collapsed. The cut on his arm could be a defensive wound.

Indeed. I'm eager to see what the full chapter 4.5 will look like with half of the second chapter reworked. As for the first chapter, it was like watching a remake of your favorite old movie with all these modern effects, a new movie, but still the same one you liked before.
I suggest that you use a save and start at chapter 4.5 now, and then wait for the full 4.5 chapter to release if you want to play from the start. That's because the full release of 4.5 come with the partially reworked chapter 2
Are you using an old walkthrough mod? That'll cause the game to either outright not launch or sometimes cause it to end at whatever update a mod was intended for.
