You almost certainly installed it into the wrong folder. See the installation section in post 1 how the folder structure should look like. Maybe your archive extractor created a folder with a certain name for your "convenience" which contains the extracted files and folders. In that case, move those out of there and where they belong.
A clean install is always better after a mayor update, but I think you will be fine this time by simply coping the new mod over the old one.

You mean the sound that comes with the *<some_sound>* tags in dialogs? Like the *Keuch* thing? That's not possible otherwise because internally it just plays the _keuch.mp3 file. I tried to make those gender independent as much as possible, but it's the same for all characters.
If you mean the instructions for the optional content, they are still there (in the optional content section). I don't think there was a special option for the main mod.

Can you elaborate some more on the "strange stutter"? Smoothing helps with the older animations which have less frames per second. You need to find your compromise.
Maybe setting the SMOOTHING parameter to 0. Or there might be an option to disable/enable hardware acceleration.
This is a mod thread, not a Win support thread, sorry. Maybe you get more support in the game's main thread. If it works without mods installed, you could ask for support on Kompassproductions.com. But since Win 7 is somewhat outdated...
The numbers are not a mod thing, they introduced them with the new update to indicate that there are more than one angle/flavor of a scene available.
Many of the features can be disabled in the mod's configuration menu. No, it's not possible to get only sounds, but you can disable most of the other stuff.
