It's a porn game, played by large majority of men, and a large majority of men love lesbian and FFM and not FMM. It's just logic. As I said before, to be equivalent, the MC must be in a sexual relationship with BOTH of them, otherwise, it's not the same. If you want to switch genders, you have to make everything else the same.

Yes, if the scene was tied to the MC being a manwhore and keeping other relationships a secret from Sis, then this would be a fitting punishment scene. Or a fitting gateway to an MFM threesome if they were both open about being able to see other people.

If the scene happened even if the MC chose Sis as his only LI, then I would be angry if the game didn't let me find out about it and break off the relationship. Forcing me to play through the game knowing that the LI cheats on the MC and you can't do anything about it, now *that* would be NTR.
