IN terms of what I want to see as mentioned by kinglionheart there are a few things:
Online, you are right, there's a lot that could be done there. Hidden sex there would be the way to go, just different scenarios, in particular behind the back stuff. Lots of different costumes.

Real Life: Inoda vs Kirito outside of sex. I want Inoda to start to prove he's better boyfriend/future husband material than Kirito in various ways instead of just sexually. Really have more POV from Asuna's perspective as she realizes that she is falling for Inoda as a whole rather than just sex with him. Make it a very slow emotional descent to join the physical.
Oh, and more sexy times in the real world - dates, foreplay, kissing. Just because you've reached the sex stage once is no reason to not go back and do more things. This can tie into the above, with Asuna realizing she enjoys more just being with Inoda rather than just sex. And when it comes to sex, there's obviously the whole condom/pregnancy scare type stuff to play with.

Gameplay: I'd like to see some more branching scenarios. Maybe a Suspicion rating, where if you want you can guide Kirito to find out the truth over time, and see how he handles it from there. If Lisbeth and other characters are coming in, then maybe Kirito could find comfort with them, breaking up with Asuna?

Which leads into, to repeat, endings: These are the ways I would like to see the game end: One, Kirito finds out. Two, he doesn't, and Asuna stays with him, either because some part of her still loves him or just enjoys the guilt and thrill of cheating behind his back. Three, Asuna is pregnant and leaves him. Four, Kirito breaks up with him, gets with another girl, but Asuna is fine with that and pregnant. IN all endings, Asuna stays with Inoda alone. Please no sharing or whoring out, or, if it must be in the game, let it be avoidable via gameplay.

How about literally the exact opposite. Asuna becomes so cock obsessed that she realizes no relationship could ever fill the void between her legs. Ends up pregnant but doesn't know who's it is so she convinces Kirito and Inoda that they're both the father.

Lastly it's an NTR game, where every girl in it who has a boyfriend is either cheating on him or about to. Only reason to get Kirito a new girl would be so she could cuck him too, that's pretty well the rules of the universe here.

Suspicion rates is such a pain in the ass. And having a ton of alternate ending so I replay the game sucks ass. A couple is fine i guess, im only playing 1 then unlocking the recollection room. But if the game makes me feel like I need a half dozen save files to track all the routes its just too much of a hassle.

if I had to bet, I would say that when sex between Inoda and Asuna becomes recurrent, they will have sex in different ways and everywhere, this based on the author's previous games. While I do like your ideas, we have to keep our expectations in check.

I personally would love to see Inoda and Asuna on a proper date. It is the date Kirito and Asuna would have had if Inoda did not interfered. It ends with them in the hotel of course. But Inoda winning Asuna with more than his dick is not likely since that is not what people come for NTR and honesty I doubt the dev knows how to write that without copying other stories.

The suspicion meter and more ending would be hard to code. No impossible but not a good use of the dev's time and resources. As some have mentioned, people don't want too much game in their NTR game. I personally in the minority and like the battle gameplay. Mostly for the battle harassments but do l find it boring to go from point to point to continue the story. He is a sole indie developer so don't expect too much.

Currently, there is no unified standard for NTR, at least no law has stipulated it. You cannot convince me, and I cannot convince you. Everyone has their own preferences and expectations, and no single work can please everyone.
As for this game, the character settings of Inoda and Asuna have already hinted at the direction of the story. From my personal understanding, Asuna's "love" for Inoda is inevitable, or to be more accurate, not only her body but also her heart will be taken by Inoda. It's somewhat similar to the NTR route in Spiral Legend V.
Vanilla? To some extent, yes. But it's based on a twisted love and a conspiracy. Even if it's gorgeous and charming, it cannot change the reality that it is an evil flower. Asuna may eventually learn the truth and have a moment to mourn her once pure and innocent self and immaculate love, but she will ultimately fall into Inoda's embrace.

Yeah, I also tend to prefer villains like Inoda (who do more of the "ugly bastard" style) over villains who are "handsome young men" or "muscular men". Even if it does go the boring one man route that isn't going to bother me one bit. Every update comes with a full gallery save so I could just use that and treat it like the soulless fap simulator it'd end up as then
