It really all lies in how it is implemented and balanced. It doesn't have to be "every 5 minutes". It could be that if you didn't eat over a whole day then you'd go to hungry. X days more and you'd become starving and so on. On the other hand, if get enough food regularly then you'd progress towards being more nourished.
Or, if you really don't like a challenge; then perhaps the dev could have an easy mode.
I agree that it takes thorough balancing and great implementation. But it doesn't have to be as sophisticated as the one in LonaRPG. Just having a system that would make you feel pressured to gain money which would allow you to survive could be nice. It's one of those exceptional games where hunger system is not an annoyance that hinders enjoyment.
In Lona, hunger is one of the core mechanics, it was there from the beginning, it gets adjasted and rebalansed to this day and a LOT of other mehcanics are closely tied to hunger.
So if someone wants to implement hunger into the game that had not even a hint of it, the entirety of core mechanics need to be rebalanced or remade entirely only to PREVENT hunger from entirely ruining balance or even worse - rendering game unplayable.
You played first Fallout? Do you know about hidden timers in it? You still enjoyed the game, yes? What if those timers were set 10 times faster? Perhaps you are right that it isn't suited for this game. If it isn't, then there should be something different that would pressure Honda to take some of the harsher decisions.
