Good suggestions, but a couple of these aren't suited to the type of game I'm interested in making.
The routes aren't so different that I can't afford to branch out early. Any "branching out" I've done so far just affects the individual quests and character relationships. The endings you get at the end will be determined primarily by the actions you take at the final stage. But will also be minorly influenced by some of your previous choices and Tiffany's afflictions. I'm not worried about how I'm going to end the game. Since I could do it at any time, even as early as the next update if I wanted to. To me it's all about how much content I can put into the rest of the game.
1. Grinding is out. Not what I'm interested in. The game can feel repetitive enough as it is without adding additional, artificial grind. Not saying there won't be gameplay segments, but I'm making a porn thing here, I don't want to make people who just want that (the majority) to have to sit through hours of grinding to get all the scenes.
2, 3, 4. Planned to some extent.
5,6. There will be one difficulty. The base game will be very hard if you want to beat it without sex scenes. The game is about conserving ammo and items & looting rather than raising stats through grinding. Likewise, I'm not going to hide unique monsters behind difficulty walls.
7. I plan on adding new outfits eventually. Those outfits just wont be featured in the story/sex-scenes as Tiffany will be naked for those anyway. There will already be heaps of variations for different levels of pregnancy. In any case, patrons will choose what they want to focus on in future updates. But eventually, it will have everything that I plan for it to have.
