Should mean that the tentacle suit takes priority on death, then.
So as long as you have it on, you'll probably get that scene.

after reaching the black area, go south, and keep going south till you reach a drain
after you interact with it, the suit latches on to you

it's only used for taking the suit off with the gargoyle's "help"

the tentacle suit scene is triggered by getting KOd while wearing it
