Having the right people review it, basically. It is entirely case by case and at the mercy of the individual(s) reviewing it and their personal standard, not a set guideline to be followed across the board.
Ending 2 is too much for my corrupted heart, holy shit this is wholesome for a porn game, makes me wonder if there are others that have this high quality of art, and the puzzles are kinda fun too ngl (and are there others like this one?).

Since 1.04a has been posted now, here's my modified copy of the main swf file. Same as I did for previous versions, I've enabled dev mode and added a few additional colors for the princess's underthings. Dev mode unlocks everything and enables some extra hotkeys.
The devmode hotkeys are probably the same as in the previous version, including "End" to toggle dev mode and "Insert" to toggle visibility of some meters and buttons:

one thing thats been bugging me, I know HLB has made 3 games, 2 of which we don't talk about here, but is there a fourth that I don't know about? Keep seeing HLB using what looks like Mikan for To love-ru, is it just fanart or is there something else?

That's supposed to be his "magnum opus". And I said supposed because that game doesn't exist yet. The dev talked, and this was like a year ago, on his blog, that the reason he created HLB in the first place was to gain enough experience to create a game using that character.
Lost Life was just a test, that was a complete success, for the mechanics he wants to add to that game. So yeah she will be in a game, just not for the time being.

3 - 4 Hearts, Cure her <best ending IMO
4 - Use Collar after 3-4 hearts, cure her (wait until she is orgasming, stop what you are doing by clicking away from her, hover over > side of screen, click collar)
5 - same as 5, just don't cure her
