I played a game exactly like that but I can't remember the name of it.
As for the sub aspect, it is something that I have noticed that many games are always forced or "Its the only 'option'" by giving illusion of choice. There never really is a game where the MC does the change on themselves willingly without the influence of someone else goading them or saying "Hey, this fetish is cool check it out". The unrealistic nature I feel is because many don't know how to make the self change look natural if not by outside influence in an extreme way.
However the subtle hint of Katie is that she wanted a sub femboy toy which is what she was trying to change you into which you learn later on in the auction and it wasn't because she wanted to actually help you. This little sneaky detail is something I did like because it wasn't obvious at first when she was 'helping you' be more fem and more like trying to help you gain more money due to the mob asking for it and you yourself pursuing a way to gain more income. It is still forced of course and would be nice if it wasn't but it was still something neat to see that there was a hidden agenda outside of what Nov was trying to do.
For Todd, he is all over the place like you mentioned and it is by far the more inconsistent when it comes to what he wants and doesn't want. Romantically you could invest yourself in him only and he will still cheat on you with Olivia, Becky or even Lexi and the only way to make him devote himself to you is to guilt trip him and make him understand why you are working at the cauldron. My first play of this game, he was a bit of a creep chasing after you and I assumed he was gay. But it seems he is more attracted to female traits than anything else which is why he goes after Olivia and Becky which means his 'gay' tendencies are a means to his end goal of the girl he wants. Even pursuing susan if you are not going into the Queen of Spade route with her, after a threesome both susan and Todd would seem to pursue each other but won't progress unless you allow it showing Todd just wants a piece of girly ass than an actual romantic relationship. The Pregnancy's thing is his kink and like you mentioned before he has daddy issues and since he likes the whole preg and motherly tits he might have some repressed mother issues too since he likes the natural mother milf look and dislikes the fake tits that his own mother has. Todd is very mixed and things do need to be cleaned up for him or his family story more fleshed out and seen into as a big side story following the main story if you are pursuing him or jack... Curious if Todd and Mother but that's just me
Cade... He needs more but aside of being the more reasonable and normal compared to Todd there isn't much there and if you yourself are following with Susan the Queen of Spade route, going for cade doesn't make much change or do anything for your own progression in the REP org or give Cade pause and see his point of view of it; if he is on board and likes the idea, disgusted and you have to convince him or lose him, or he himself would gain a harem along with you. Susan also has no comment if you pursue cade, a congratulation for committing to REP or bringing in cade into the relationship. And even when pursuing cade, if you choose to be fully a part of REP, why can't you also corrupt Olivia while also gaining cade for the same purpose. Many possibilities with cade aside from football star who wants to be in a play.
Out of many games that include M/M and Sissification this is by far my favorite to recent date and I hope to get more. But one can always look and want more to their own preference, I just want MORE content even if it means fixing some story or character flaws because I kinda doubt i'll get a game close to this one.
