For being dom you need:

Scene with oral does not add sub or dom points. So you don't have to skip it to be dom. But in the next update this scene will have a different endings, depending on the points gained during the dialogues.
So if you skip this scene, you just miss some content.

The last option never shows for me.

Agreed, femdom can be a turn on sometimes, but i do agree it can be a bit meh since that them is in almost every porn game as well as other themes if done right, but you do bring up in having both sub or dom points, would be fun with Kenzie, and the other gals to have that kind of points during each event.

After shoot the third guy (really begining of the game) the dialogues diasapear and we go choice to choice whithout understanding one thing....

Just restart the game. If it doesn't help: Main menu -> Preferences -> Language -> English
