Well you're right, but I think that crypto currencies are already good enought, maybe just not use bitcoin. If you by the right amount a crypto before paying you don't pay much more.I am afraid most money processing sites and payment gateways will have similar rules. Even those which are porn-friendly like CCBill.
The reason is, these are the rules the card companies enforce on them. One can probably operate more-or-less staying under radar, but the risk will always be there. The more levels are there, the bigger a chance.
For example, I believe both Visa and MasterCard don't allow any porn site, to directly accept their cards. But via certain payment operators they kind of let it slip (like CCBill or Verotel). But they still don't allow certain things, which even these companies check on the site they provide payments for. But I've seen sites, selling credits - totally valid, and those credit scan be then used somewhere else, to buy not-approved content. Naturally it's inconvenient and adds cost - both to sellers and for buyer (devs/patreons).
In future there's the chance what crypto-currencies offer, but right now their price is too volatile to work reasonably. But perhaps once this hypes dies out, we will see a new dawn (Personally I wish this boom crashes the sooner the better)

Only biggest brands can use paypal. Paypal forbids porn and he would lose his acount with all funds for TOS violation. That's why Patreon is special
