They did exactly that for a long time now. But this works only for so long. At one point you have to come forward and do the final big step.
Yeah, but it's to late now. Fingers cross that the game survives this decision.

They tried that. They spent several months laying the groundwork for it. Unfortunately, a pregnancy mechanic in a porn game is a bull in a china shop just breaking all the things.
Adding pregnancy is usually a six-month black hole and subsequent months of bug fixes judging by older games. Degrees of Lewdity has a better coding team than most games do, but it also has the kind of massive bloat that only a game that has spent eight years in alpha can accumulate. Let's give it a couple more months before giving it up for dead.

And when it's four months of waiting for something that you couldn't give two shits about? That will require months upon months of bug fixes after it finally appears? That will need to be accounted for in every subsequent update thus slowing development even further? But, hey, it's not like vrelnir could have avoided it after he spent years promising his patreon backers that he would do it eventually. All we can do is hope that it doesn't turn all of the game's spaghetti code into a flying spaghetti monster.

to be fair the dev said they've been working on it SINCE THE FUCKING GET GO.
like they could've done this massive update in implements as they did the regular update, instead of waiting for the perfect time to do it.
