It looks more like a ketchup stain than blood. The game does not bring enough money so he has to eat fallen leaves with ketchup.

So does Kaitlyn have the baby in the coming update? She doesn't look pregnant in these previews.

My guess is this is a flashback to how she got pregnant.
That makes sense. I guess part of the plot is for us to guess who the father is.
