I regret to say that this game stopped to please me.
Each subsequent version has more and more errors, and the developer disposes of those who report these errors and still only persuades to become his patron, which for me is rather pathetic, because I remember a few months ago he did not have such an ignoring approach to players, who reported bugs here in the game,
Now, reading his answers, you can clearly see that only cash counts, because if someone reports another bug in the game and gets the answer that there is a bug fix on the site patreon (but of course you must have a subscription of at least $ 10 or more to have access to this bugfix or a newer version of the game that the bug fixes).
That's false, my friend. For one reason: I lowered the money needed to get the top tier on Patreon from 20$ to 10$, I give a Steam key for free to 20$+ patrons which means you can get the FULL GAME with all updates and then delete the subscription and never pledge again.

And with the Steam discounts you can get the full game for 12$ without pledging. Not bad for someone for whom only cash counts uh?
I wish the developer, of course, good luck in further development of the game, but I also recommend to consider if only players who are his patrons or also bought a version of the game on steam they count for him.
Moreover, one thing has indeed changed: I am now developing games for a living, meaning that I feed myself and my family with this, and while I do check this forum for feedback, as I always did, I have less time to reply to long posts as I had earlier, because I want to deliver the FULL GAME on December, and not keeping "milking" patrons as so many other devs do. This project will be finished, but in order to do so I need to work on it, not spending time replying to bug reports made on the forum (I read them, I solve them, I publish the fix, what else?)
And yes, people who actually pay for my games allow me to keep doing this job, so I have a special attention to them. But if I ignored this community, why post the news here as well? I have plenty of social media channels to do so...
The amount of content has improved as well: 4 exams every month, a complete story arc (I am ending every relationship in game and the main story will unravel on October), a hundred new pictures every update, new sex animations... Then again, where do you see "cash only" written on it?
Finally, about the "porn game"... General Practitioner was NEVER intended to be a PORN game, but an ADULT game which is very different. Post #14 on this thread (FOURTEEN!) already noted that:

I'm sorry other people/website (even Patreon) say it's a porn-content game, but I never sold it for being a porn game, so I haven't changed anything about it, I always said GP is an ADULT game.
As I said many times, I want to improve and be a better developer, and posts like yours help me a lot in doing it, when they don't say something that is not true (I'm also thinking about going to a "per-release" pledge against a "monthly" one, so I wouldn't call it "cash only attitude" either). What I saw is that I don't get replies to what I post and say here like I did in the past, while games which are abandoned (Big Brother anyone?) are still in top view.
