From memory the pregnancy stamp is obtained instantly when you enter the bedroom and interact with the girl, you prolly managed to get yourself a bugged stamp.
How do i get Squidmaid pregnant? I've yet to get the [???]Stamp from her

Open the menu (Cogs in top right). There's a little window where the squid maid will be working, and behind her is a sticky note. You can't access it during sex/when she's present, but if you just open the menu when she's talking to you (The small talk after "training" is done), she won't be there and you can read the notes. (Reading all of them will give you 3 pregnancy points instead of the usual 1, I think)

It is possible to get 3 pregnancy points instead of 1 but I don't think it has to do with the notes, I know cause I haven't even beaten the boss rush and I have the 3 pregnancy points, I think it has to do with the special paper
