Main and Second wife confessions are from early on (R15), but the actual marriage happens in R24.
Concubine's "Confession" is from R22, nothing happens in R24 regarding this one, but you will get new hentai scenes for her later on.

It wouldn't really make sense if we had everything available in one playthrough as most of the content is different consequences from your earlier choices.

If you're gonna play this game from the very start, it's long enough to satisfy your hunger for games because of the split choices you need to make for you to achieve the story path you wanted to choose or if you just want to complete the gallery..
