That's a bit of a contradiction. "Do what you want, except if people don't like it." But also, if the fetish is the whole point, why should it be avoidable? I mean, don't get me wrong, I know about catering to people and how to get the largest audience, but there's a difference between the "largest audience" and "target audience". If someone wants to make a game, based on a lesser known franchise, specifically for that fanbase, he can be liberal with the story and change key elements to cater to as much people as possible, but piss off the hardcore fans, or stay true to the source material and only get a small fanbase. So, he either does what he wants to do, or he does what people want. In most cases, you can't have both.
