Most of the game descriptions attempt to give you a certain sense of interest-building idea of what you'll play. It doesn't really mean you'll be able to avoid the issue, just that you'll play a man trying to do so. For you, the player, it's pretty much a rhetorical question

It's this little thing called the law. It's illegal in Japan to do a lot of the explicit stuff. Can't remember just how deep or complex the requirements for uncensored stuff is, but it's enough that pretty much everything from softcore LA to hardcore ultra-niche dojins get some kind of censoring.
Do you need to do horrible mosaics that cover half the screen, or put a huge white blob? Not really, again, it depends on what you're censoring: For cocks, you can get away with little strips of censorship around the frenulum and the head, and women can go from just the opening and the clit, all the way to "if it can have a single pubic hair there, that squared inch of skin must be covered"

Most games are translated by smaller groups or individuals what do it for free and they won't bother with the art.

Honestly what I find immensely stupid is that they censor it outright, like on the design itself. In 3D games? Many don't even bother to make a vagina and a dick. On 2D? The mosaic is there from the get go, like they leave the hole there and they just mosaic filter over it.

That's a sign that the Japanese devs don't give a single fuck. They don't give a fuck about the game and they don't give a fuck about you. Because if it were me and the laws in my country forced me to censor? I would at the very least make it able to be decensored with reasonable effort for the rest of the world. Hell, I would probably make a patch myself and VPN through VPN through Tor and then another VPN and release it on the torrents, just to make sure it couldn't be traced back to me. I'm 100% sure and certain the weebs will catch-on and redistribute the patch to proper channels.

But that would mean effort, and to actually give a fuck about my cheap cash grab full of bait and blue-balling. Which isn't something you'd find often in Hentai games.
