So, after stumbling into the bad ending, I opened the walkthrough and started a brand new game. I made sure to follow the Karen/Nikki path to the letter, all the way through. Still got the bad ending again. What gives? Can that path be achieved?

Yes, YOU did something wrong, no doubt about it !!! I played allso it from start to finnish (had bad ending without WT), and ALL 4 routes witch I played, works just fine !!!!
