You have to move main plotline. Go to the palace and talk with with Guard in a basement. Wait a couple of days so he will reject you and then you can go to the sewers. Doctor scenes will trigger when you lose the fight with anything there.

For real now, whether we get a beta version download link or not, Dynamite is planning to add even more content to the major release next week. This dev should open a porn game academy or something, there are a lot of other devs needing a lesson or two.

Is not possible in that version, but there's a new release (0.4) that may be. Your save will be broken, so I would wait to continue the gameplay if this was my first try.

Not possible at all even with a very high exhibitionist stat (I tried with 100...)

Are you talking about the new version 0.4?Because didn't the main stat for that change to "Shame" instead of Exhibitionism?
