Hmm...I haven't seen that yet. I assume it only happens if you trigger it from a previous ending or something.
If you are talking about the very beginning after you picked your character class and who I'm assuming is Node asks if your sleepy No there is no change to the gameplay if you say yes or no to being sleepy.

How to get the full heroines ending exactly?
I'm pretty sure that I got all the heroines and things stay the same after I defeated Dinah

There is no ending with full heroines, but if your trying to get the ending B with Node which requires:
Raise the Covenants of specific heroines(Node, Bill, Dodo, Shisha,
the Duchess, the Hatter, the Mock Turtle, Wolris, Tweedle Twins) to Lv3,
then make love with Node and after it chose to give up on your search. Your not required to do much more, at least that's what the English guide says.
Anyone else still hoping for a black souls III, or really any additional game by the developer?

I have found a very neat secret. THERE IS A "SECRET" Hidden folder in the bonus folder of the game. Anyone knows the password to it? I NEED IT

This is a semi secret scene that can only happen during a new run. Before beating the headhunting rabbit there's a chamber that has four paths, north heads to rabbit, south is where you came from, west is a dead end, and east is guarded by a headless knight. Behind him is a room that holds a stand with an item on it. If you pick up the item it says 'Drink Me'. Use that item and you will shrink down. Then head to the back of the room and go through the hole there that you can now go through since you're small. You'll be in the bloody pond area head over to the spore forest and you get a scene based on what you chose for alice. Daughter = blowjob, sister = footjob, mother = nursing handjob
One thing to note is seeing that scene counts as 1 progress for the purpose of if you can see RRH or when JubJub ruins Wonderland.
