It's not that rare. I've had it several times since. The main issue seems to be that there are two variations: one with panties and one without. In the one with she makes it really clear. In the without, she basically just says "come to my room tonight for something fun". So if you've only seen the one without panties, it can make it seem like it's something else.

I think I'm stuck in chapter 2. Extra Naughty 2 doesn't work, I managed the first star to appear, but missed the second star. Then all stars disappeared and even if Chloe was spanked 3 times a week already, the first star doesn't appear any more. I attached my save file.

You don't ignore her but you must deny her 3 plus times in a roll for it to happen.

You must deny Rachel help at least 3 times and keep denying her until it happens.
You might want to tell her to stop asking and then go talk to her and help her then start the denying her and see it that works.
