I had this question awhile back since that image has been in the CG files for a long time, but it turns out Kate doesn't ever sit naked on the couch; it's just a layer render and whatever outfit you've selected for Kate is added. If you look through the CGs you'll see cutout renders of her outfits.

I have not been able to achieve the all-three-in-the-shower scene, either, and I've finished all of v.015 online. Since it's only one image and not a "scene" I wonder if it's actually implemented in the game?

Did the developers say anything about when they'll start up the male protagonist storyline? Will it be after they finish the female's if at all?

The male protagonist storyline is abandoned, I guess. Instead, you have Max with "Big Brother"-Content.
