Seriously? The US not puritanical? If you honestly think that, you obviously don't live here. In the US to this day nudity is bad and shameful, sex is dirty and sexuality in general is something to be ashamed of. Some blue states are less ignorant, but the majority of the country is still affected by mainly religious ignorance, leading to absolutely toxic views on sex and sexuality being promoted as "virtuous".Its not that the country is puritanical, its that there's nothing at this time to be gained by challenging the minority of people that will get upset once they learn Incest is being funded by PayPal. Who at Paypal is going to risk the personal and professional fallout that will certainly arise when they stand up and say "We've decided that incest is OK!". Its easier and safer to disallow it, and until we start dropping anvils on knee jerk reactionaries - better business to deny it and just keep our heads down.
When it comes to Patreon not allowing incest content, it's just a safe play more than anything I think. In this cancel culture we're living in, no company based in the US is willing to risk it by allowing taboo content and then having the executives and ownership labeled as taboo or incest promoting.
