Can anyone tell how do the initial choices affect gameplay, specifically experience, sensitivity, secret and core trait/title gain ? Does difficulty matter ?

Difficulty makes the fights more longer and you'll need more grind or better strategy with engrams and objects.

I mean, story stuff will always be hit and miss with people. It's all subjective. People like this game for more than its rather generic magical girl story, even though this one too has some nice twists to it that you don't run into all that often, if at all.
So all in all, I'd like to say yes, but I have NO idea what you disliked about the intro of all things, so I can't answer that question.

Yeah, the next part prepares you for a math exam, and then the game really kicks into high gear as it starts teaching you about the wheat policies of Roman Emperor Tiberius.

Dumb question but how do you make the Enemies actually do Lewd stuff to her? more often than not they just smack me a bunch

this only happens if the enemies have an icon with red hearts above them. when a gray hand appears along with the hearts, the next turn they will make an attempt to grab you and do lewd things

In addition of TheodoreBear answer, they do more through your progress in the game. In first dungeons they only do groping and kissing, forced blowjobs. From the Sealed Streets (chapter 3 if I recall correctly) they do vaginal and anal.
