a) the way you wrote it you ask what the difference between version 5c and 5c is
a1) that the difference between 5b and 5c is bugfixes, so you just need 5c
b) Mega 1 and Mega 2 are just mirrors of the same file

Well.. that all depends on Icstor. But it could be about 2-3 months between updates. And as this game was recently updated last weekend from V0.5B to V0.5C, to correct a render error and animation glitches(?) then it'll probably be around Jan next year. Unless he releases it earlier. Patreon followers get first dibs on this game along with voting on who is in the next update along with other "perks.
Check the front page for any, and all, information relating to this game.

Update 0.4 was also a double character uptdate and it didn't took ages, so I assume, 0.6 will also be playable in in two or maximum three months.
Maybe ICSTOR had begun working on the secondary character yet, so he doesn't have to wait for the poll results. And I don't think that it will be a character that has something specifically to do with Caroline as for Liza's story in 0.4 wasn't connected to Caroline at all.

Someone already asked about NTR stuff on ICSTOR's official discord channel, and I think he said that he will never include NTR in Milfy City because his high paying patrons wanted to "kill" him just because of a conversation between Caroline and mc. So, just imagine what would happen if he includes NTR
