Hmm... I hadn’t really thought about it that way.
I did mention to patrons about a month ago that I was working on an NTR scene…
What I don’t really get is — it’s a hidden scene that only triggers if you specifically go find a side character and choose the NTR option. It’s basically like the scene doesn’t even exist unless you go looking for it.
My game still mainly focuses on netori and blackmail-style domination, so I’m honestly wondering… is this really something worth getting that angry about?
Am I just lacking empathy here?

Some actual (hopefully constructive) criticism and perspective:
As a quick perspective from a players side as to why so much anger. Because its easy to look at it and go 'It's just one scene. It's avoidable.
I personally support alot of different titles. Some of these are to the amount of $100's if not more over the course of a games development cycle.
I also have a personal rule, I never support NTR/Sharing/Swinging/Cuck or anything that involves 'other dicks'.
So lets say that I believed your statement that you would never add NTR to your game and decided to support it - I would now be in a situation where my $upport had gone towards you making NTR content.
Once could even argue that, you had obtained funding under false pretenses (which is probably not a good thing legally) as deception to obtain funding is fraud, and criminal in most places.
There is also the typical direction that these things can (and often do) tend to go. Which is that 'Vanilla' or Harem or 'non other dicks' players will drop the title, and in order to recover revenue or replace the now missing funding a developer will turn to the players that remain, who are either NTR Neutral or Pro-NTR. Which means more gets added. This stems from a simple fact that NTR players can and will play 'vanilla games' but Vanilla players will not play 'NTR games'. The result is that once you start down that path, it tends to the 'more and more ntr like content'.
Additionally there is the optics of it. When alot of us who have been in the community for a while see statements like : "It's only one scene" We immediatly think 'So far". When you say "It's just an out of the way, avoidable scene" We think "that still took Dev time that could have been spent elsewhere". Look at it from our point of view, some of these games take years to complete, spending additional time adding content like NTR or whatever, increases the development time for the game overall.
Even if every NTR scene(s) you ever added totalled only 1 months development time over a 3 year game. Thats still an addition 1 month of 'Patreon Cash' that your asking from me and everyone else who activly support(ed) you. Which doesn't exactly incentivise players to jump on the 'monthy support' bandwagon.
Ultimatly, its your game, you can do what you want. But the other side of that coin, is that, as a supporter, I will do what I want. I cannot (and don't want to) speak for everyone who activly contributes to games. But when I see that you lied about this game and broke the never adding 'ntr content'. My thought process goes like this.
I will never support a game that Developer makes during the games development cycle ever again. Period.
The only way I will every support anything that developer makes is if the game is complete, I play it to completion, liked it, and it had zero 'other dick' content.
Even in this case, I will donate like $10, which is far less than $5-10 per month for 3+ years.
There also the 'moral' question (Wierd topic for a porn/pirate site I know).
You broke your promise (and one might assume your original vision for the game) for some cash. Is that the kind of person I really want to support?
I think thats enough ranting from me. I am not trying to bully/beat a dead horse. You asked if you were "just lacking empathy". Well, only you can answer that. Best I can do is try to provide you with a players (or at least my own -as a players- perspective.
