An event needs to trigger before you can start collecting. He will jack off on Serena when she turns her ample butt to him. I don't know the trigger, but you could try different clothes and character states, although it might even be tied to story progress. Maybe just approach him again?
How do you unlock the true end? The recollection room does not give any hints and I finished the main quest and most of the side quests (for some reason the last priest side quest is not getting marked as completed, even though I did the confession event).
The priest side quest didn't get marked for me either, but after getting the true end you get the option to unlock the full recollection room which does mark it and few of the bad ends that don't get marked either. About the true end, go up the stairs from the teleporters and talk to the main guy, there will be a marker above his head.

For the second I have high lust 150 and 25 sanity.
After I have done this it unlocked both in the Recollection Room.

Guys, I can't start training because the Explorer's ID is not in my inventory. This is when Aria does get me the Explorer's ID after vaulting back over the counter, but the ID is never placed into my inventory. Hence I can't make it to the Nursery - the guard won't let me in. Please help.
I can't say for certain because what you're talking about is in the early part of the game and all my saves are from near the end but I don't see an explorer's id in my inventory. So make sure you're not missing something else. Either ways, there is a debug menu, you can probably find the proper trigger which will grant you access or give you the item, but it could break your game, I'm not really knowledgeable about that aspect. Make a seprate save, or copy your save elsewhere and give it a try maybe.
Yes, there is a slight variation. It might be a lust/sanity requirement that triggers it, she either takes off her pants or her top. Otherwise it's just a single scene with a couple of parts.
