Even if your pretty sure it's not on your end it wouldn't hurt to rule things out.
Also if you need help from the forum some basic information would increase the chance of someone helping you out.

Operating system. (Is it updated?)
Ram?
Is it a Intel/Nvidia combo or AMD. (Are your drivers updated?)

I believe this game requires an incest patch so if you don't apply it, your mom and sister will be old friends or something like that.
Patreon has rules about what content types any product (game) funded via its service may contain, incest is one of the proscribed content types.
Hiding proscribed content types within a product isn't allowed, thus why the creator doesn't "use a switch" to turn such content on/off. Nor can the creator supply "proscribed content enabling patches", thus why such patches are supplied by someone else.
